Current product or service vendors: If you currently provide products or services to Ascension Health (Health Ministry or hospital), please answer “Yes” to the question below. You will be asked to provide basic information that is needed because of a new payment system.  This site will provide you the opportunity to enter the information necessary to receive payment for the products or services you provide.  Here is a list of the mandatory information being requested:

• Name

• Address

• Email

• Phone Number

• Federal Tax ID number – Required only if your business is registered as a Corporation or Partnership

• Social Security Number – Required if you do not have a Federal Tax ID number

• W-9 Tax Forms – Required for those with a Federal Tax ID number and a Social Security Number

Please note: This is a secured website. Information will not be shared outside of Ascension Health.  There is no fee for using this site.   Entering information into this site will not result in any changes to the mutually agreed upon payment for the products or services you provide.
